Notification Package

Depending on the agency's requirement, you will receive a notification package with some or all of the following items:

• Confirmation letter
• Service bill
• Medical history questionnaire
• Pertinent medical records
• Authorization forms (as needed)

The contents of the package are for your review prior to the scheduled appointment and include all items required to conduct the examination. If an examinee comes to your office asking you to complete any additional forms, please contact QTC immediately.
