A patient initially presented with a hemoglobin A1c level of 17. Following personalized care delivered through the MHC, the provider modified the treatment plan and, within 4 months, the patient’s A1c dropped to 7.5 and no longer required insulin.
The patient reported feeling empowered and optimistic, sharing that follow-up visits are now a positive experience—no longer associated with “bad news” about his diabetes.
